We are seeking an experienced full-time Family Support Worker to work term-term plus 25 days. The role of the Family Support Worker at St Lucy’s School is to support the students and their families to reach their goals and access appropriate services, to increase their independence and participation in the community.

We provide a holistic education, offering outstanding academic programs taught by special educators with the support of our dedicated therapy team. </p><p><b>ABOUT THE ROLE </b></p><p>We are seeking an experienced full-time Family Support Worker to work term-term plus 25 days. The role of the Family Support Worker at St Lucy’s School is to support the students and their families to reach their goals and access appropriate services, to increase their independence and participation in the community. The Family Support Worker collaborates closely with students, families and staff to advise, advocate and provide information to enable students and their families to make informed decisions and to support them in gaining access to and navigate systems and services. </p><p><b>KEY RESPONSIBILITIES </b></p><ul><li>Respond to referrals for support for specific students and their family from relevant classroom staff and senior staff including the Principal, Deputy Principal and the Heads of Curriculum and Pedagogy. </li><li>Participate in planning and implementing appropriate supports for students and their families as part of the Tier 3 Wellbeing Team. </li><li>Maintain a database of potential services to provide informed advice and information to families about accessing suitable external services. </li><li>Support families with building networks of support and case co-ordination including linking them to and liaising with government agencies and external service providers, including DCJ, NSW Health, NDIA and NDIS services, therapists etc. The work is family–centered, which means direct engagement with and support of family members as determined by the requirements of each case. </li><li>Educate families so that they can advocate for their child and with a focus that over time they can independently build on networks and navigate support systems and services. This includes providing workshops for families in relation to navigating the disability sector and other services. </li><li>Work with the students and families in relation to access to appropriate supports through the National Disability Insurance Scheme (NDIS). </li><li>Contribute to the development, continuous quality improvement and evaluation of disability support services and systems for students and their families keeping up to date with research and best practice and raising systemic issues in relation to the NDIS scheme or other systems with the School Leadership. </li><li>Maintain progress notes and details on student and family files which remain the property of the School. </li><li>Collaborate and consult with other members of the St Lucy’s team to develop strong, coordinated support for St Lucy’s students and their families. </li><li>Participate in case conferences with relevant staff, family members, Psychologist and therapists to develop programs for individual students as needed. </li><li>Develop and maintain positive relationships between the School and its various communities. </li><li>Facilitate and support events for families, providing opportunities for them to network and support their well-being for example Men’s and Mum’s evenings. </li></ul><p><b>ABOUT YOU </b></p><p>The School&#39;s core values are Joy, Community, Courage and Truth.
